Community Health Workers (CHWs) are one of the five traditional health worker roles recognized by the state of Oregon. As trusted members of their communities, CHWs play a vital role as frontline public health workers, bridging the gap between individuals, families, and essential health and social services. They build strong, culturally responsive relationships and deliver individualized care to address health inequities. Our team specializes in supporting the Latine community by addressing various social determinants of health. We offer services, including resource navigation, assistance with Oregon Health Plan (OHP) enrollment and support, SNAP benefit guidance, and emergency services to help individuals and families thrive.
